Consent forms for other to access your medical record and/or prescription collection

If you would like a carer, family member or friend to have access to discuss your  medical record, we will need signed consent from you.  Please find two forms, one for over 18s and one form for 16 to 18 year olds.

This also applies for patients who would like a carer, family member or friend to collect their prescriptions from us.




Bromley residents can now access online STI testing, regardless of age.

For patients who are 24 or under (only).  If you have changed partners in the last 6 months or had unprotected sex, you can have a National Chlamydia Screening Programme kit (NCSP kit) which tests for chlamydia and gonorrhoea. Alternatively, please go to the Sexual Health Bromley website here where you will be able to order a postal kit to your home.

For patients of any age; if you think you are asymptomatic and would like to test for chlamydia, gonorrhoea, HIV and syphilis, please go to where you can order a postal kit to be delivered to your home. Asymptomatic patients who attend the Sexual Health (GUM) clinic at Beckenham Beacon will not generally be seen in clinic; they will be offered a self- sampling kit to be taken home.
