New Patient Registration

To register with our practice please follow the links below to complete the online registration form and questionnaire.

After you have completed  the forms you must send them back via email (due to COVID-19) to the surgery, within 7 days with one form of identification from both sections (1 & 2).

Acceptable identification includes:

Section 1

  • Passport
  • Driving licence
  • Birth Certificate
  • Rough sleepers’ identity badge
  • Hostel Registration/mail forwarding letter


Section 2

  • latest council tax letter
  • utility bill (no more that 1 month old)

Until we have seen your identification you will not be registered at the practice.
