The practice nurses give advice for foreign travel and administer most vaccinations for patients registered with the practice. The practice does not issue NHS prescriptions for drugs for use when travelling abroad.
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include the country and areas within country that you are visiting to determine what vaccinations are required.
Travel Health Questionnaire
To help us offer the appropriate advice, you must complete and hand in the travel health questionnaire form to reception at least 6 weeks before you travel. This will give the nurse enough time to order your vaccinations and book you an appointment. The appointment booked is for you and NOT your family, if all your family need vaccinations for the same trip you must complete one form per family member and let the receptionist know the appointment is for a family.
Please print and complete the Travel Risk Assessment Form below and hand into the surgery at least 6 weeks before you travel:
Our nurse is unable to give advice if you are travelling to several destinations in the same trip, we advise you to go to a travel centre where you will receive expert advice and care.
There is further information about countries and vaccinations required on the links below or visit the National Travel Health Network and Centre (NaTHNaC) website.
It is important to make this initial appointment as early as possible – at least 6 weeks before you travel – as a second appointment may be necessary with the practice nurse for certain vaccinations. Some vaccines have to be ordered as they are not part of our general stock. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Most travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. Our charges are reviewed on an annual basis.
